Skybreak Therapy offers speech-language, occupational, and physical therapy for kids from birth through 18. Parents who suspect something might be off with their child's development are in a hard spot. They do not know whether what they are seeing is normal, or whether a full evaluation is warranted.
The practice's website could explain what services existed, but it could not answer that question. That is a lost lead and, more importantly, a family that does not get help.

A parent selects their child's age and answers milestone questions drawn from the relevant disciplines. The tool scores the responses and points them to the right next step. It works as both an intake front door and a lead-generation tool: the parent gets a real answer, and the practice gets a qualified inquiry instead of a cold form fill.

Fourteen clean, non-overlapping age bands from birth through 18, with milestone questions that match the standards clinicians use.

Responses are scored and routed automatically: on track, a free screening, or a full evaluation recommended.

Every check uses clear, parent-friendly language and calm colors, so a worried parent feels guided instead of alarmed while they answer.

We worked directly with their web developer to embed the tool properly on their existing website, instead of handing over a link and walking away.
We refined the tool with the practice over several rounds, matching the clinical standards their therapists work from, and turned every round of feedback around fast, often the same day.
